I release SR Sensory Gym, its owners, employees, volunteers, and affiliates from claims arising from injury, illness, accident, or property damage.

Medical authorization

In an emergency, I authorize SR Sensory Gym staff to obtain necessary medical treatment and accept financial responsibility for services provided.

Health and safety

I confirm my child is able to participate and agree not to bring my child if they are experiencing symptoms of illness.

Facility rules

I agree to follow posted rules and staff instructions. I understand failure to follow guidelines may result in removal without refund.
